Claim form
Complete the claim form and mail
it to the specified address in the claim form. You will receive a confirmation page and quote number upon submission.
Please print it out for your records and reference your quote number for all questions relating to this promotion.
Cash back
Once your documentation is verified, allow 4-6 weeks from the time your claim is complete to get your cash-back check.
